Before submitting your application, please review the following instructions:

Complete all fields marked with an asterisk (*). All other fields are optional.
Be sure to include both a valid phone number and email address so we can contact you.
Select the program you are requesting:
- A. Rotational Shelter
- B. Prevention Rental Assistance
- C. Diversion Move-In Assistance

Please note:

When calling us, please leave only 1 voicemail message. Do not leave multiple messages. Leaving multiple messages fills up our machine and makes it harder for us to get back to people. If you leave 1 voicemail message, we will return your call.
Families must add all household members in order to apply. Parent(s) must have at least one child under the age of 18 in order to apply. Parent(s) must have legal guardianship of their child(ren) in order to apply.
Our Intake Coordinator will contact you within 5 business days of receiving your completed application (depending on the volume of intakes).
Completing an intake application does not guarantee funding or placement in our shelter program.
Financial assistance through our Prevention and Diversion programs is a one-time service.
