Before submitting your application, please review the following instructions:

1. Complete all fields marked with an asterisk (*). All other fields are optional.

2. Be sure to include both a valid phone number and email address so we can contact you.

3. Select the program you are requesting:

   • A. Rotational Shelter

   • B. Prevention Rental Assistance

   • C. Diversion Move-In Assistance

Please note:

• When calling us, please leave only 1 voicemail message. Do not leave multiple messages. Leaving multiple messages fills up our machine and makes it harder for us to get back to people. If you leave 1 voicemail message, we will return your call.

• ​Families must add all household members in order to apply.

• Our Intake Coordinator will contact you within 5 business days of receiving your completed application (depending on the volume of intakes).

• Completing an intake application does not guarantee funding or placement in our shelter program.

• Financial assistance through our Prevention and Diversion programs is a one-time service.
